Job description

INTRODUCTION
Under the supervision of the Practice Manager, the Nutrition Trainee provides nutritional counseling for all primary care patients to achieve health goals and manage medical diseases such as hypertension, hyperlipidemia and diabetes. The Nutrition Trainee will assist with patient outreach, scheduling, nutrition education initiatives, and value-based care activities.

MAJOR DUTIES/ESSENTIAL FUNCTIONS:

  • Ensure compliance with federal, state, and MMA policies, procedures, and confidentiality requirements.
  • Assist in developing, implementing, and evaluating nutrition education programs and outreach initiatives.
  • Assist with scheduling nutrition appointments, and support patient outreach and follow-up.
  • Assist with food insecurity and community resource coordination.
  • Proactively monitor patient progress on health related goals.
  • Participate in quality improvement and population health initiatives.
  • Participate in required trainings, professional development activities, and staff meetings.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Bachelor's in Nutrition, Dietetics, Public Health, or related program; and on track for certification in nutrition/dietetics
  • Familiarity with the nutritional interpretation of labs.
  • Strong interpersonal and communication skills.
  • Interest in ambulatory care and preventive medicine.
  • Proficiency with Microsoft Office; EHR experience preferred.
