Job Summary and Responsibilities

As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. • Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care.
  • Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.
