Position Summary
We are seeking an experienced Rehabilitation Nurse (RN or LPN) with a strong background in post-acute and long-term care rehabilitation to join our clinical leadership team. The ideal candidate is a hands-on nurse leader who excels in coordinating rehab care, supervising CNAs, collaborating with therapy teams, and delivering impeccable customer service to residents, families, and facility partners.
This role is critical to ensuring excellent clinical outcomes, smooth unit operations, and a positive rehab experience for every resident we serve.
Key Responsibilities
Clinical & Rehabilitation Care
  • Provide skilled nursing care to short-term and long-term rehabilitation residents
  • Monitor residents' progress related to mobility, ADLs, pain management, wound healing, and functional improvement
  • Collaborate closely with Physical Therapy, Occupational Therapy, and Speech Therapy teams
  • Accurately document assessments, care plans, progress notes, and changes in condition
  • Identify early signs of decline and communicate promptly with providers and leadership

Leadership & CNA Management
  • Supervise, mentor, and support CNAs to ensure high-quality resident care
  • Delegate assignments appropriately and ensure CNAs follow care plans and rehab goals
  • Address performance concerns professionally and in accordance with facility policy
  • Promote teamwork, accountability, and respectful communication on the unit

Customer Service & Professional Representation
  • Deliver impeccable customer service to residents, families, therapy teams, and facility leadership
  • Serve as a positive representative of the organization at all times
  • Communicate with empathy, professionalism, and clarity
  • Support admissions, discharges, and family education as needed

Compliance & Safety
  • Ensure care is delivered in compliance with state regulations, CMS guidelines, and facility policies
  • Maintain infection control, safety, and fall-prevention standards
  • Participate in audits, surveys, and quality improvement initiatives as assigned

Qualifications
  • Active RN or LPN license in the state of North Carolina (unencumbered)
  • Rehabilitation nursing experience required (post-acute, skilled nursing, or LTC rehab units)
  • Demonstrated experience supervising CNAs
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication, leadership, and customer service skills
  • Ability to work independently and as part of an interdisciplinary team
  • Familiarity with EMR systems preferred
  • Reliable transportation and flexibility to work assigned shifts

Experience
  • Skilled Nursing Facility (SNF) or Long-Term Care rehab experience
  • Experience with short-term rehab, orthopedic recovery, or post-hospitalization care
  • Charge nurse or lead nurse experience
