Location: Port St. Lucie, Fort Pierce, Vero Beach, and surrounding areas.

About the Role

As a Baylor Weekend Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes throughout the West Palm Beach to Delray Beach service area. You'll work independently in the field while receiving ongoing support from our Clinical Managers, QA team, and office staff.

Key Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits.
  • Complete comprehensive assessments and develop individualized plans of care.
  • Provide skilled nursing services, including medication management, wound care, patient education, and chronic disease management.
  • Accurately complete OASIS and clinical documentation in our EMR in a timely manner.
  • Collaborate with physicians, therapists, and interdisciplinary team members.
  • Educate patients and caregivers to promote safety, independence, and improved outcomes.
  • Adhere to all Medicare, ACHC, and agency policies and procedures.
  • Active RN license in the state of Florida (required)

  • 1 year of Medicare home health experience strongly preferred

  • Experience with Wellsky/Kinnser preferred

  • OASIS experience preferred

  • Current CPR certification

  • Valid driver’s license, reliable transportation, and active auto insurance

  • Strong clinical assessment and time-management skills

  • Comfortable working independently while staying connected to a team
