Location: Sebastian, Vero Beach, Ft. Pierce
Position Type: Part-Time / PRN 

***Sign-On Bonus*** A $1,000 sign-on bonus is offered for this position, payable within 90 days of employment. To be eligible, the employee must remain actively working and in good standing with the company at the time of payout.

About the Role

As a Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes, following physician orders and Medicare guidelines. You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and office staff.

Key Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits

  • Complete comprehensive assessments and develop individualized plans of care

  • Provide skilled nursing services including medication management, wound care, patient education, and chronic disease management

  • Accurately complete OASIS and clinical documentation in our EMR in a timely manner

  • Collaborate with physicians, therapists, and interdisciplinary team members

  • Educate patients and caregivers to promote safety, independence, and improved outcomes

  • Adhere to all Medicare, ACHC, and agency policies and procedures

Qualifications
  • Active RN license in the state of Florida (required)

  • 1 year of Medicare home health experience strongly preferred

  • Experience with Wellsky/Kinnser preferred

  • OASIS experience preferred

  • Current CPR certification

  • Valid driver’s license, reliable transportation, and active auto insurance

  • Strong clinical assessment and time-management skills

  • Comfortable working independently while staying connected to a team
