Position Overview

Mohawk Trail Counseling Center is seeking a dedicated Licensed Professional Counselor (LPC) to join our innovative team in a fully remote capacity. This position offers the unique opportunity to provide high-quality mental health services from the comfort of your home while reaching a diverse client base in Grants Pass, Oregon. Key Responsibilities

As a Licensed Professional Counselor at Mohawk Trail Counseling Center, you will be responsible for:

  • Conducting comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations to guide treatment plans.
  • Providing brief, focused interventions to patients seeking mental health support, ensuring they receive timely and effective services.
  • Collaborating closely with primary care providers to identify and manage mental health and psychosocial issues, facilitating a seamless care experience.
  • Developing ongoing follow-up plans to monitor patient progress and adjust treatment approaches as needed to optimize outcomes.
  • Identifying at-risk patients and devising proactive strategies to prevent the escalation of their conditions.
  • Maintaining accurate and detailed clinical documentation in compliance with agency policies and the Federally Qualified Health Center (FQHC) guidelines.
  • Working with patients to develop self-management skills, empowering them to take an active role in their treatment.
  • Engaging in constant communication with primary care providers to ensure coordinated care and address any evolving patient needs.
  • Contributing to team discussions regarding behavioral health services and the effectiveness of existing programs.
Patient Population / Client Focus

Your work will primarily focus on adult and adolescent patients facing a range of mental health challenges, including anxiety, depression, and co-occurring disorders. You will have the opportunity to work with individuals from diverse backgrounds, ensuring you understand the social and economic factors that may impact their mental health and well-being. Your expertise will not only improve patient health outcomes but also enhance their quality of life.

Qualifications

The ideal candidate will possess the following qualifications:

  • A minimum of a Masterโ€™s Degree in Clinical Counseling is required.
  • A current and active LPC (Licensed Professional Counselor) license is essential.
  • Knowledge of mental health assessment, treatment planning, and various therapeutic methodologies, including family therapy and group therapy.
  • Understanding of DSM-IV diagnostic criteria and behavioral therapy techniques.
  • Strong verbal and written communication skills, demonstrated ability to maintain clinical documentation.
  • Capacity to make sound judgments and decisions based on professional experience.
Preferred Experience

While it is not required, bilingual proficiency in English and Spanish is preferred to better serve our diverse clientele. Other desirable experiences include familiarity with community resources and an understanding of the impact of physical and mental health conditions on social dynamics.

Compensation and Benefits

We offer a competitive salary range of USD 87,000 โ€“ 105,000 per year, commensurate with experience and qualifications. In addition to salary, we provide a generous sign-on bonus, health benefits, and professional development opportunities to help you advance your career as a counselor.
