Job description

Provide Hands-On Support for Daily Function in Patients Homes
@Home Therapy is seeking a Certified Occupational Therapy Assistant (COTA) to provide in-home therapy support to adult patients in the Astoria area. This role focuses on assisting individuals with functional daily living tasks and reinforcing treatment plans developed by licensed Occupational Therapists.
This position is well-suited for clinicians who enjoy hands-on patient care in a flexible, field-based setting.

What You’ll Be Doing:
Implementation of Treatment Plans:
  • Carry out therapy interventions designed by the supervising Occupational Therapist.
  • Support patients in practicing functional daily living skills.
Patient Monitoring & Feedback:
  • Observe and report patient progress and response to therapy.
  • Communicate changes or concerns to the supervising OT.
Caregiver Support:
  • Reinforce caregiver training provided by the OT.
  • Help maintain consistency in therapeutic techniques between visits.
Documentation:
  • Complete visit notes using a web-based EMR system.
  • Ensure timely and accurate recordkeeping.

Work Structure & Environment:
  • Flexible Scheduling: Choose your availability and manage visits independently.
  • Urban Community Coverage: Provide care in Astoria and surrounding Queens neighborhoods.
  • Team-Based Clinical Support: Work under the supervision of licensed Occupational Therapists.
  • Adjustable Caseload: Opportunity to scale visits based on preference.

Qualifications:
  • Active New York State COTA license.
  • Experience with adult or geriatric populations preferred.
  • Strong communication and follow-through skills.
  • Ability to work independently while following clinical direction.
  • Comfort using electronic documentation systems.
