The Role

The Occupational Therapist is a licensed healthcare professional responsible for the evaluation, development, implementation, and oversight of individualized occupational therapy plans of care. This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.

Core responsibilities include:

  • Conducting comprehensive occupational therapy evaluations and re-evaluations
  • Developing, implementing, and modifying individualized plans of care
  • Delivering skilled therapeutic interventions
  • Assessing and recommending adaptive equipment
  • Monitoring patient progress and modifying treatment plans
  • Reviewing patient medical history and conducting patient interviews
  • Evaluating patient home or workplace environments
  • Leading discharge planning and transition-of-care coordination
  • Communicating evaluation findings and progress updates
  • Educating patients, families, employers, and caregivers
  • Supervising and directing Occupational Therapist Assistants
  • Providing oversight to rehabilitation aides, students, and volunteers
  • Completing timely, accurate, and compliant documentation
  • Participating in care conferences, quality assurance initiatives, and collaborative planning
  • Maintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable Arizona state practice standards
  • Treating all residents with dignity, respect, and compassionate professionalism

Physical requirements include the ability to assist with patient transfers, upper extremity handling and positioning techniques, mobility assistance, repositioning and supporting patients with or without assistive devices, prolonged standing, walking, bending, reaching, kneeling, and stair navigation. The ability to assist in resident evacuation during emergency situations is also required.

What You Bring

  • Current, active, and unrestricted Occupational Therapist license in the State of Arizona
  • Graduate of an accredited Occupational Therapy program
  • Successful completion of the National Board for Certification in Occupational Therapy
  • Current CPR/BLS certification
  • Ongoing compliance with continuing education requirements
  • Experience in a skilled nursing, post-acute, hospital, or related medical setting
  • Excellent verbal and written communication skills
  • Composure and professionalism in dynamic, fast-paced clinical environments
  • Strong organizational skills and the capacity to make independent clinical decisions
  • A team-first mindset and a genuine passion for skilled nursing and post-acute care
