Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. THCC is proudly recognized as a Level 3 Patient Centered Medical Home (PCMH), the highest level of recognition attainable for quality care.
We boast two primary care locations and 6 school-based health centers providing quality value-based care for over 20,000 patients. We provide services in Primary Care, Infectious Disease, Substance Use, Integrated Behavioral Health, Dental Services, Womenโ€™s Health, and Pharmacy.

Position Summary:
The HealthCare Connection (THCC) is seeking a School-Based Behavioral Health Provider for a unique opportunity to serve children, adolescents, and families within our School-Based Health Centers. This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the lives of underserved communities through integrated care.
As a School-Based Behavioral Health Provider, you will:
  • Deliver brief, solution-focused assessments and interventions aimed at functional restoration.
  • Collaborate closely with primary care providers to develop and implement clear treatment plans.
  • Provide behavioral health services, including crisis intervention and substance abuse screenings.
  • Educate and consult with the primary care team on mental health, chemical dependency, and behavioral medicine.
  • Support patients and families with self-management, behavior change, and harm reduction strategies.
  • Identify referral resources and coordinate care for patients requiring specialized services.
  • Maintain thorough documentation in Electronic Medical Records (EMR).
Key Responsibilities:
  • Conduct comprehensive bio-psychosocial assessments addressing mental health, substance use, co-occurring disorders, and medical needs.
  • Manage psychosocial aspects of chronic and acute diseases in collaboration with the primary care team.
  • Be immediately available during working hours to assist with crisis interventions and psychiatric emergencies.
  • Provide individual, group, and family interventions as needed within the school setting.
  • Use electronic tracking tools to monitor treatment outcomes and ensure effective care coordination.
  • Educate staff, patients, and families on mental health best practices.
Qualifications:
  • Masterโ€™s degree in Clinical Social Work, Counseling, or Psychology.
  • Licensed in the State of Ohio (LISW, LPC, LPCC, LMFT).
  • Three (3) or more years of experience in behavioral health, preferably in an integrated primary care setting.
  • Strong knowledge of population-based healthcare, behavioral medicine, and evidence-based treatment models.
  • Experience working with diverse, multicultural populations.
  • Bilingual (Spanish/English) strongly preferred.
