Key Responsibilities

• Conduct comprehensive psychosocial assessments through direct interviews, observational techniques, and meticulous review of medical records to accurately evaluate the emotional, social, and environmental factors influencing each patient’s recovery.
• Provide empathetic counseling and emotional support to patients and their families, helping them navigate the complexities of adjustment, grief, and coping mechanisms as they confront life-changing situations.
• Collaborate closely with interdisciplinary rehabilitation teams, contributing your insights to the creation of tailored treatment plans that holistically address patients' physical and psychosocial well-being.
• Assist patients in accessing a wide array of community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring they have the necessary tools for successful reintegration into their communities.
• Deliver education and advocacy for patients and families regarding post-rehabilitation services, fostering a supportive network during the transition from care back into daily life.
• Participate actively in discharge planning efforts, working alongside healthcare professionals to facilitate smooth transitions for patients returning to their homes or to alternate care environments.

Patient Population / Client Focus

Your work will be fueled by a commitment to a diverse patient base characterized by unique challenges and needs. Patients may have chronic medical conditions, neurological disorders, or are recovering from significant orthopedic surgeries and strokes. Your role will not only involve addressing their psychological needs but also aiding them through their emotional recovery, encouraging resilience, and maximizing their functional capabilities.

Qualifications

• Master's degree in Social Work from an accredited program.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• Exceptional verbal and written communication abilities, combined with a strong aptitude for building trusting relationships with clients and families.

Preferred Experience

Compensation and Benefits
