Job description

Home Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency on a PRN basis and serve existing and new clients across our Southeast Ohio region, primarily serving Jackson, Vinton, and Gallia Counties.

More about the role:

  • This is a PRN / As Needed schedule, with hours and shifts varying week to week, including some weeks having low or no hours at all.
    • Candidates seeking long-term employment are preferred, however short-term or temporary employment will be considered.
  • Yes: this position has the potential to grow into a consistent Part-Time (Hourly) or even Full-Time (Salary) position if desired - not required.
  • PRN RNCMs MUST work a minimum of 4 shifts hours per month to remain active.
    • It is strongly preferred that PRN RNCMs work at least 1 shift per week.
  • Typical travel distances include a 30-minute radius daily + up to 60-minutes on occasion. Additional hours are available if willing to travel up to 2 hours as needed (optional, not required).
  • Door-to-Door mileage reimbursement provided.
  • At least 1 year of experience in case management is required.
    • 1 or more years of experience in home health care, wound care, lab draws, and Oasis assessments is a plus.
  • Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and reviews.

Job Summary
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Administrator, or Clinical Administrator/Director of Nursing


Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management required, with home health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
