• Collaborate with primary care practitioners to identify and address mental health and psychosocial concerns, enhancing the overall effectiveness of the patient care team.
• Conduct comprehensive patient intakes, such as diagnostic and functional evaluations, leading to the development of customized therapy plans tailored to individual client needs.
• Provide emergency mental health interventions for patients in crisis, ensuring timely and effective support.
• Advise primary care providers on appropriate treatments for patients, facilitating referrals to specialized mental health services as necessary.
• Perform ongoing follow-ups to monitor patient progress, evaluate treatment efficacy, and adjust therapeutic strategies as warranted.
• Support the establishment of relapse prevention techniques alongside patients, empowering them to maintain their progress.
• Engage in proactive identification of patients at risk of deteriorating health issues and contribute to preventative program development.
• Coordinate and supervise the delivery of mental health services, collaborating both within and outside the primary care setting based on patient consent.
• Document patient interactions comprehensively and accurately, keeping clinical records current and compliant with regulatory standards.
• Maintain regular communication with primary care providers to ensure collaborative treatment planning and continuity of care.
• Implement behavioral change methodologies to assist patients with co-occurring physical health issues, promoting healthier lifestyle choices.
• Equip patients with self-management strategies and educational resources, fostering their active engagement in their treatment journeys.
