This position partners with patients, families, providers, and interdisciplinary teams to ensure safe,ย timely, and well-coordinated transitions from hospital to post-acute settings, with a strong focus on reducing avoidable readmissions and improving patient outcomes.ย ย ย ย

Whatย Youโ€™llย Doย 

  • Temporary, part-time position (*potential to become permanent)ย leadingย comprehensive, patient-centered discharge planning beginning at admission and continuing through transition of care.ย 

  • Identifyย high-risk patients early and implement proactive strategies to support safe discharge planning and reduce adverse outcomesย 

  • Perform holistic assessments of clinical needs, functional status, support systems, home environment, and social determinants of health (SDOH)ย 

  • Develop and manage individualized discharge plans aligned with patient goals, clinical needs, and available resourcesย 

  • Coordinate post-acute services and community resources, including home health, skilled nursing, rehabilitation, hospice, and outpatient careย 

  • Proactivelyย identifyย and resolve barriers to discharge, including placement challenges, insurance authorization, equipment needs, and social or financial concernsย 

  • Support informed patient choice by educating patients and families on post-acute care options and available servicesย 

  • Facilitate seamless care transitions, ensuringย timelyย communication with post-acute providers, outpatient teams, and community partnersย 

  • Participate in multidisciplinary rounds, providing clear updates on discharge readiness, barriers, and next stepsย 

  • Ensureย accurate,ย timely, and compliant documentation of assessments, plans, and discharge outcomesย 

  • Coordinate durable medical equipment (DME), follow-up care, and appointments prior to discharge to promote continuity of careย 

  • Support patient and caregiver education, reinforcing discharge instructions and promoting understanding of care plansย 

  • Monitorย utilizationย and length of stay, supporting efficient throughput andย appropriate level-of-careย decisionsย 

Whatย Youโ€™llย Need

Required:ย 

One of the following:ย 

  • Licensed Registered Nurse (RN)ย 

  • Licensed Practical Nurse (LPN)ย 

  • Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW)ย 

  • Experience in care management, discharge planning, case management, orย utilizationย managementย 

  • Strong understanding of care transitions, post-acute care services, and community resourcesย 

  • Excellent communication, collaboration, and critical thinking skillsย 

  • Ability to manage multiple priorities in a fast-paced acute care environmentย 

Preferred:ย 

  • Prior experience in acute care case management or hospital-based care coordinationย 

  • Case Management certification (e.g., CCM, ACM)ย 

  • Experience working with complex patients, high-risk populations, or social determinants of health barriersย 

  • Familiarity with regulatory and accreditation standards related to discharge planning and transitions of careย
