Job description

RN โ€“ Home Health (URGENT)

Facility: First Choice Home Health (Boise, ID)
Location: Boise, ID
Setting: Home Health & In-Home Hospice Care


Contract Details
  • Duration: 13 Weeks (or 26 weeks option)

  • Start: ASAP

  • Schedule: Mondayโ€“Friday

  • Shift: 8:00 AM โ€“ 5:00 PM (8-hour days)

  • Hours: 40 hrs/week

  • Productivity requirement: 30 points/week


Pay
  • $54 โ€“ $56/hr

  • Weekly Pay: $2,160 โ€“ $2,240/week


Required Credentials
  • Active Idaho RN License required

  • BLS Certification required

  • Home Health RN experience required

  • OASIS documentation experience required

  • Homecare Homebase (HCHB) experience required

  • Strong case management skills required

  • Ability to meet productivity targets (30 points/week)

  • Valid driverโ€™s license and reliable transportation required (home visits)


Responsibilities
  • Provide skilled nursing care in patient homes

  • Complete OASIS assessments and documentation

  • Develop and implement patient care plans

  • Coordinate care with physicians and care teams

  • Educate patients and families on care plans

  • Manage caseload and meet productivity requirements

  • Provide hospice and home health support services
