Registered Nurse Diabetes Educator

Location: Rockford, Illinois

Pay Rate: $2,200 To $2,400/week

Contract: 13 weeks

Schedule: Monday–Friday, Day Shift

Total Working Hours: 40 Hours/Week

Experience: Minimum 2 years required

Setting: Clinical Diabetes Education and Nursing Care

About the Role

We are seeking an experienced Registered Nurse Diabetes Educator for a 13-week assignment in Rockford, Illinois. This role focuses on providing compassionate diabetes education, patient advocacy, and coordinated nursing care in collaboration with a multidisciplinary diabetes management team.

Responsibilities

• Provide diabetes education to patients and families living with diabetes mellitus

• Develop and support individualized patient care and education plans

• Collaborate with physicians, nurses, and interdisciplinary team members

• Administer medications and treatments according to nursing standards and policies

• Document care accurately in accordance with standards of care

• Serve as a resource for patient education materials, programs, and care guidance

• Support patient safety, patient experience, and performance improvement activities

Requirements

• Active Registered Nurse License in Illinois and Wisconsin Required

• Graduate of an accredited nursing program Required

• CDCES Certification Required

• BLS/BCLS Certification Required

• Minimum 2 years of nursing and/or diabetes education experience required

• Strong communication, patient education, and advocacy skills

• BSN Preferred

• EPIC experience Preferred
