Job Title: LTSS Service Care Manager/Case Worker
Location: Hybrid – Must reside in Del Rio, TX
Contract Duration: 6 Months (Contract-to-Hire Potential)
Department: LTSS (Long-Term Services and Supports)
Work Schedule: Monday – Friday, 8:00 AM – 5:00 PM (1-hour lunch)
Travel Requirement: ~75% field travel within the MRSA West area (3 days/week), 2 days remote
Interview Format: 1 Microsoft Teams Video Interview (Camera On)

About the Role

Seeking an experienced LVN or equivalent professional experienced working with Aged, Disabled and Blind population, to join our LTSS team as a Service Care Manager. This hybrid role blends field-based assessments with remote documentation and service planning for members receiving long-term care. This is a unique opportunity to make a real difference for aged, blind, and disabled populations while enjoying the flexibility of both in-field and remote work.

Key Responsibilities

· Conduct member assessments at home using standardized tools (e.g., H2060 form)

· Evaluate complex and high-risk members and recommend appropriate care plans

· Support or facilitate the development of service authorization for personal attendant services

· Complete accurate documentation and submit authorizations/denials within 3 business days

· Educate members, caregivers, and families on long-term services, procedures, and community support options

· Collaborate with providers, specialists, families, and community organizations

· Maintain timely, detailed, and compliant records in the electronic care management system

· Meet quality and turnaround benchmarks, including a 92%+ quality assessment score

· Act as a member advocate to ensure access to services and optimize outcomes

· Attend meetings, shadowing, and onboarding activities per Centene’s standards

· Travel locally (~75%) with reimbursement for mileage; company laptop and phone provided

Qualifications

Required:

· LVN license (Texas) or bachelor’s degree in healthcare, social work, or 3+ Years of Experience working with aged, disabled or blind population.

· 2–4 years of related experience

· Must reside in or near Del Rio, TX

· Experience with the aged, blind, or disabled population (5+ years preferred)

· Proficiency with Microsoft Office Suite (Outlook, Word, Excel)

· Reliable transportation for in-field visits
 

Preferred:

· Bilingual – English/Spanish

· Experience in LTSS, case management, care coordination, or community health

· Background in social work, nursing, or human services
