Full Office | Monday–Friday, 8:00 AM–5:00 PM
Salary Range:
($34.66 - $55.4582) $72,096 – $115,353 annually
Job Overview

CalOptima Health is seeking a compassionate and experienced Care Manager to join the OneCare team temporarily. This role is responsible for coordinating comprehensive care management services for members with complex medical, behavioral health, and social needs. The ideal candidate will have strong clinical assessment skills, experience in managed care or case management, and the ability to collaborate with interdisciplinary teams to improve member outcomes.
Key Responsibilities
Care Management (85%)

  • Conduct standardized health needs and health risk assessments.
  • Perform comprehensive clinical assessments including physical, functional, psychological, and social evaluations.
  • Complete post-discharge assessments following hospital or emergency department visits.
  • Develop individualized care plans with measurable SMART goals.
  • Monitor member progress and revise care plans as needed.
  • Coordinate services with physicians, specialists, caregivers, community agencies, LTSS, and behavioral health providers.
  • Facilitate referrals to community resources and support services.
  • Participate in interdisciplinary care team meetings.
  • Conduct in-person visits in hospitals, skilled nursing facilities, member homes, and other care settings as needed.
  • Maintain timely and accurate documentation within care management systems.
  • Advocate for members to ensure access to appropriate services and resources.
Administrative Support (10%)
  • Support departmental goals and quality initiatives.
  • Document case interventions according to organizational protocols.
  • Track and report productivity metrics.
Other Duties (5%)
  • Complete additional projects and responsibilities as assigned.
Qualifications
Required
  • Bachelor’s degree in Social Work, Psychology, Gerontology, Public Health, Social Science, or related field.
  • Minimum 5 years of clinical experience and/or managed care experience.
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Ability to travel locally for member visits and care coordination.
Preferred
  • Licensed Vocational Nurse (LVN), Licensed Marriage and Family Therapist (LMFT), or Master’s degree in Social Work, Gerontology, Public Health, or related field.
  • Certified Case Manager (CCM) certification.
  • Experience in behavioral health or substance use case management.
  • Bilingual in English and one of the following languages: Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese.

  • Experience working in fast-paced healthcare environments.
