Description:
The Visit Nurse helps provide continuity of care by performing nursing visits for patients as delegated by the Case Manager RN.
The Case Manager RN is responsible for the overall planning and coordination of care for a caseload of patients.
The Visit RN is a member of the interdisciplinary team providing nursing care in patients' residences as prescribed by the physician.
Care must be compliant with applicable laws, regulations, and policies of the agency.
The Visit RN will keep the Case Manager RN informed about patient progress, recommended actions, physician orders, and plans for upcoming visits.
Individuals in this role are expected to apply their knowledge of nursing principles and practices.
Demonstrating problem-solving skills and the ability to make independent decisions in complex work situations is essential.
