Role Summary

The Care Navigator supports patients by coordinating referrals, scheduling appointments, and resolving barriers to care. Operating in a fully remote capacity, this role conducts telephonic and digital outreach, educates patients on next steps, and ensures referral loop closure within defined SLAs. Success requires empathy, attention to detail, and a proactive approach in a fast-paced, virtual environment dedicated to patient advocacy and operational excellence.

Shift: 11:00 AM – 7:30 PM EST

Key Responsibilities

● Patient Engagement & Advocacy: Conduct timely outreach via phone, text, and email

to schedule appointments and provide culturally responsive education to reduce

no-shows.

● Referral Management: Prioritize referral tasks to meet SLAs, coordinate with specialty

providers, and track statuses to ensure successful loop closure.

● Data Integrity & Compliance: Document all interactions accurately in EMR/CRM

systems, adhering to standardized templates and HIPAA regulations for audit-ready

records.

● Operational Excellence: Monitor personal queues and dashboards to prevent delays

while following internal QA workflows to ensure high-quality patient outcomes.

Pay and Benefits

The pay range for this position is $20.00 - $20.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
