POSITION SUMMARY:
St. Joseph’s Home Care is a not-for-profit home and community care organization and a member of St. Joseph’s Health System (SJHS).  The Data Analyst - (DA-C) supports strategic, operational, and quality improvement activities across the home care business operations. This role blends business analysis, data analytics, performance reporting, evaluations, application administration, workflow optimization, and other data reporting responsibilities to strengthen service delivery, operational efficiency, and organizational decision-making. This role also supports continuous quality improvement strategies and action as necessary. 
The DA-C collaborates with senior leadership, program managers, clinical teams, finance, quality and digital/IT partners to provide insights, design, analyze, and improve business processes, develop and analyze reporting solutions and reporting systems, and enhance performance measurement tools. This role works with Leadership in ensuring high-quality organizational decision-making and service excellence. 
The role provides analytics, reporting, and performance measurement expertise across Home Care’s clinical and community-based programs - transforming operational, clinical, financial, and human resources data into timely, actionable insights that support informed decision-making. The role is responsible for developing and maintaining key performance indicators (KPIs) that support organizational performance measurement, quality improvement initiatives, and strategic priorities. 
The role supports Home Care leadership by providing end-to-end analytical support utilizing a variety of business analysis methodologies, tools, and software, collaborating closely with Home Care Program Managers, Finance, and the broader Data Analytics team to support activities related to specific programs, departments, and operational functions. 
CORE DUTIES AND RESPONSIBILITIES
•  Support strategic initiatives, organizational goals, quality improvement plans, and business development activities. 
•  Conduct business process reviews, workflow mapping, process re-engineering, and operational assessments. 
•  Design, develop, maintain, and enhance dashboards, scorecards, reports, databases, data collection systems, and performance measurement tools. 
•  Analyze and present metrics, trends, KPIs, business intelligence, and operational reports for leadership. 
•  Perform data collection, validation, analysis, interpretation, and root-cause investigations. 
•  Support Requests for Proposals (RFPs), grant submissions, business cases, growth initiatives, partnership opportunities, and building business cases 
•  Coordinate application configuration, testing, upgrades, implementations, maintenance, and user access. 
•  Serve as a subject matter expert for corporate applications, EMR and reporting systems. 
•  Troubleshoot system, workflow, reporting, and data integrity issues. 
•  Collaborate with IT and digital teams on system enhancements, upgrades, and data integrity. 
•  Develop evaluation tools, surveys, feedback mechanisms, and outcome measurement processes. 
•  Create training materials, user guides, documentation, and deliver end-user education. 
•  Conducting root-cause analysis and supporting continuous quality improvement (CQI) activities. 
•  Prepare scheduled and ad hoc reports, presentations, scorecards, and executive summaries. 
•  Monitor industry trends, best practices, and emerging technologies. 
•  Participate on committees, project teams, audits, reviews, and change management activities. 
•  Ensure compliance with privacy, confidentiality, quality, and corporate requirements. 
•  Perform other related duties as assigned. 
•  Design, build, and maintain recurring operational and executive reports and dashboards (Power BI / Excel) covering data points such as visit volumes, referral and intake activity, missed care, hours of service, and program utilization across all Home Care programs. 
•  Extract, integrate, validate, and analyze data from home care clinical and operational systems, reconciling activity data across multiple sources to establish a single trusted view. 
•  Conduct ad hoc analyses to answer program, funder, and leadership questions, presenting findings as concise briefing notes and visualizations with clear recommendations. 
•  Build and maintain cross-continuum reporting for the Integrated Comprehensive Care (ICC), SJHH @ Home, and St. Mary’s Health @ Home programs, connecting hospital discharge activity with home care service delivery and outcomes (such as readmissions, ED visits, time to first home visit). 
•  Collaborate with SJHS’s broader Data Analytics team to align data definitions and, where appropriate, leverage hospital data assets (Epic/Cogito). 
•  Support provincial reporting requirements relevant to home and community care. 
•  Provide measurement support to inform quality improvement initiatives across Home Care programs. 
•  Support costing of programs and visits, contribution analysis, and rate adequacy reviews to inform contract negotiations and service planning. 
•  Support budgeting, forecasting, and monthly variance analysis. 
•  Supporting evaluation activities including surveys, focus groups, and stakeholder feedback analysis. 
•  Provide operational support for technology, reporting, and workflow troubleshooting. 
•  Providing ad hoc and/or on-request information, data, analysis, reports etc. as necessary 
•  Presenting regular and on-going information, data, analysis, reports based on a pre-determined schedule of dates 
•  Assisting with communication materials such as reports, presentations, and program documentation. 
•  Performing other duties as required

QUALIFICATIONS
  • Undergraduate degree or college diploma in Business, Data Analytics, Health Informatics, Information Technology, Computer Science, or related field – required 
  • Master’s degree (e.g., MHI, MPH, MBA) - an asset. 
  • 3-5 years of progressive experience in business analysis, data analysis, or evaluation - required 
  • Minimum 2-3 years of experience in healthcare analytics, decision support, or performance reporting - required; home and community care, hospital, or Ontario Health Team experience - an asset. 
  • Experience with data collection design, analysis, and reporting required - required 
  • Experience with healthcare information systems, electronic medical record systems, SLQ, reporting platforms and applications (e.g., EPIC, AlayaCare) - preferred 
  • Experience delivering training, documentation and end user support - preferred 
  • Proficient in MS Office and Office 365/Teams, Excel (Power Query, pivot models, charting) and strong Power BI skills to create relevant visualizations - required. 
  • Proficiency in SQL for data extraction and transformation - required; experience with relational databases and reporting from clinical systems used in home care (e.g., AlayaCare, Procura) - an asset.
