PROCEDURE: I. Chemotherapy guidelines: A. Chemotherapy shall be prepared only by specially trained pharmacy personnel. B. Only RNs with documented competency and who have successfully completed the ONS/ONCC chemotherapy certificate course may be allowed to administer chemotherapy. C.
