Schedule Details/Additional Information:

Full time home health LPN, Licensed Practical Nurse, to support the Advocate Health at Home South East team. Normal office hours are Monday through Friday, 8:00am to 5:00pm. Rotating holiday schedule of 2 holidays per calendar year.

Pay Range:

$25.30 - $37.95

  • 60628: Washington Heights, Roseland, Cottage Grove Heights, Pullman
  • 60643: East Beverly, Beverly, Morgan Park

Schedule: Full-Time | Monday-Friday, 8:00 AM - 5:00 PM
Rotating holiday schedule (2 per year)

💰 Sign-On Bonus: Up to $3,000!

Position Overview

Join our Home Health team as a Licensed Practical Nurse (LPN), delivering compassionate, high-quality care to patients in the comfort of their homes. Working under the supervision of an RN, you'll play a key role in supporting patient outcomes, coordinating care, and empowering patients and families through education.

Key Responsibilities
  • Deliver skilled nursing care in accordance with individualized care plans under RN supervision
  • Monitor patient conditions, recognize changes, and communicate updates to the care team
  • Coordinate care with interdisciplinary team members to ensure continuity and optimal outcomes
  • Provide patient and family education on disease management, treatments, medications, and safety
  • Accurately document care to support clinical, regulatory, and reimbursement requirements
  • Participate in case conferences, team meetings, and ongoing care planning
  • Promote patient independence and engagement through education and support
  • Maintain compliance with organizational policies and quality/safety standards
Required Qualifications
  • Active LPN license in the State of Illinois (or eligible)
  • CPR Certification
  • Pharmacology certification
  • Minimum 2 years of Med/Surg clinical experience
  • Valid driver's license and reliable transportation
Preferred Experience
  • Prior home health experience
  • Familiarity with Medicare/Medicaid home health guidelines
Skills & Strengths
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and documentation abilities
  • Ability to work independently and manage time effectively
  • Comfortable using EMR systems and basic Microsoft Office tools
  • Strong interpersonal skills with a patient-centered approach
Work Environment & Requirements
  • Travel throughout assigned geographic area to patient homes
  • Ability to lift up to 35 lbs (with assistance/equipment as needed)
  • Frequent standing, walking, bending, and patient assistance
  • Work performed in patient homes and community settings
Compensation

💵 $23.60 - $35.30 per hour (based on experience)
