POSITION OVERVIEW:

The Registered Nurse (RN) provides clinical leadership and direct patient care for both the Community Health Center (CHC) and the Mobile Health Program in Alameda County. This position is responsible for delivering high-quality nursing services while ensuring compliance with California Title 22 regulations, applicable FQHC standards and best practices, supporting health plan's Facility Site Review requirements, and adherence to organizational policies. The RN plays a key role in immunization services, infection prevention, health screenings, Triage/nursing assessments and patient education. Working collaboratively with providers, clinical staff, Community Health Workers (CHWs), and community partners, the RN supports equitable access to high-quality primary and preventive healthcare for underserved populations.

This position will report to the Chief Medical Officer. This position will work closely with the Community Health Services Director/Clinic Administrator. This role will uphold and champion the agency's mission, vision, and values and contribute to a collaborative and inclusive work culture.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

Clinical Nursing Services

  • Perform comprehensive nursing assessments, triage, and patient education.
  • Administer medications, vaccines, and treatments in accordance with provider orders and standing protocols.
  • Coordinate preventive care, chronic disease management, and follow-up services.
  • Support care coordination, referrals, and patient navigation.
  • Respond to medical emergencies and participate in emergency preparedness activities.
  • Document patient care accurately in the electronic health record (EHR).

Community Health Center Operations

  • In conjunction with the Chief Medical Officer, coordinate daily nursing operations and clinical workflows.
  • Provide clinical guidance and supervision to LVNs and Medical Assistants within California scope of practice for Mobile services
  • Assist with implementation of clinical policies, procedures, and standing orders.
  • Support patient-centered, culturally responsive, and trauma-informed care.

Regulatory Compliance & Quality Improvement

  • Assist with Community Health Center regulatory surveys.
  • Coordinate preparation for health plan facility site reviews, including CDPH Title 22, Alameda Alliance for Health and other contracted health plans.
  • Oversee CLIA-waived testing quality assurance and competency.
  • Support Vaccine for Children (VFC) program compliance, vaccine storage, handling, and documentation.
  • Lead infection prevention and control activities.
  • Monitor emergency equipment and medication readiness.
  • Conduct clinical audits and participate in quality improvement initiatives.
  • Assist with staff orientation, competency validation, and ongoing education.

Mobile Health Program

  • Serve as the lead Registered Nurse during mobile health clinics and outreach events.
  • Provide immunizations, health screenings, triage, and preventive nursing services in community settings.
  • Collaborate with providers, Medical Assistants, Registration staff, Eligibility Specialists, and Community Health Workers to ensure efficient patient flow.
  • Coordinate referrals to primary care, specialty care, behavioral health, and community resources.
  • Promote health education and disease prevention within underserved communities.
  • Ensure clinical quality, safety, and regulatory compliance during all mobile health activities.

Other

  • Attend trainings and participate in events relevant to key responsibilities.
  • Track and regularly report on key metrics for the responsible function areas.

Qualifications

QUALIFICATIONS & EXPERIENCE:

  • Current California Registered Nurse (RN) license in good standing required
  • Bachelor of Science in Nursing (BSN) preferred
  • Current Basic Life Support (BLS) certification required
  • Experience with immunization programs, chronic disease management, and preventive health services.
  • Experience 1-2 years in Community Health Centers (CHCs), FQHCs, or public health settings preferred.
  • Experience with CLIA-waived laboratory testing and quality assurance.
  • Vaccine for Children (VFC) program experience.
  • Bilingual English/Spanish preferred.
  • Experience in non-profit and community-based organizations preferred.
  • Demonstrates and models excellent "customer service" orientation and ability to work with diverse populations.
  • Exercises extreme discretion with the ability to hold, balance, and transmit public and confidential information needed by various parties.
  • Proficient in MS Office and Google Workspace products.
  • Valid CA Driver's license required ability to travel to community outreach sites.

  • Review and submit timesheets by established deadlines to ensure timely payroll processing.
  • Notify supervisor promptly of any scheduling conflicts, absences, tardiness, or timekeeping issues.
  • Maintain consistent and reliable attendance to support team and program operations.
  • Follow organizational policies and procedures related to overtime, schedule changes, and paid/unpaid leave.
  • Correct and/or report any timekeeping discrepancies immediately to the supervisor for resolution.
