Registered Nurse (RN) – Corporate Clinical Operations & Multi-State Home Health

Location: Indianapolis, Indiana (Corporate Office)

Position Type: Full-Time

Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Travel: Approximately 40–60% (Multi-State)

Salary: $105,000-$125,000 annually

Position Summary

The Corporate Clinical Operations RN will be based in our Indianapolis Corporate Office and travel throughout our service areas to provide clinical support, patient care, quality oversight, and operational assistance.

When not traveling, this individual will work from the Indianapolis office supporting Quality Assurance, clinical documentation, regulatory compliance, staff education, and operational initiatives.

This position requires an experienced, adaptable nurse who enjoys balancing direct patient care with leadership and operational responsibilities.

Travel Responsibilities

Travel may be required to support our agencies in:

  • San Diego, California
  • Minneapolis/St. Paul (Minnetonka), Minnesota
  • Las Vegas, Nevada
  • Phoenix, Arizona
  • Broward County, Florida
  • Stamford, Connecticut
  • Chicago, Illinois
  • Ohio
  • Taunton, Massachusetts

Essential Responsibilities

Clinical Responsibilities

  • Perform skilled nursing visits in patients’ homes.
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, and Discharge assessments.
  • Complete accurate OASIS documentation.
  • Develop individualized Plans of Care.
  • Assess patient needs and coordinate care with physicians and interdisciplinary teams.
  • Educate patients and caregivers.
  • Ensure regulatory compliance.
  • Complete documentation within required timelines.

Clinical Operations

  • Support multiple branch locations with clinical operations.
  • Assist Directors of Nursing and Clinical Managers.
  • Provide temporary clinical support during staffing shortages.
  • Assist with onboarding and mentoring new clinicians.
  • Participate in process improvement initiatives.
  • Help standardize clinical practices across all locations.

Quality Assurance & Compliance

  • Perform chart audits.
  • Review documentation for Medicare compliance.
  • Evaluate OASIS accuracy.
  • Assist with ADR, TPE, UPIC, and medical record reviews.
  • Participate in ACHC, Joint Commission, CMS, and State survey preparation.
  • Monitor regulatory compliance and identify opportunities for improvement.
  • Support QAPI initiatives.

Administrative Responsibilities

  • Assist with policy implementation.
  • Support clinical workflow improvements.
  • Participate in interdisciplinary meetings.
  • Collaborate with leadership on operational initiatives.
  • Complete additional projects as assigned.

Required

  • Active Indiana Registered Nurse license.
  • Ability to obtain additional state nursing licenses.
  • Minimum 3 years of Home Health nursing experience.
  • Strong knowledge of Medicare Conditions of Participation.
  • OASIS proficiency.
  • Excellent clinical assessment skills.
  • Strong organizational and communication skills.
  • Experience using Home Health EMR systems (WellSky experience preferred).
  • Ability to travel extensively.
  • Valid driver’s license.
  • Ability to work independently.

Preferred

  • COS-C certification (or willingness to obtain).
  • Quality Assurance experience.
  • Clinical Manager or Director of Nursing experience.
  • Multi-state Home Health experience.
  • Survey readiness experience.
  • Regulatory compliance experience.
  • Experience mentoring clinicians.
