What You'll Do

  • Facilitate telemedicine and in-person provider visits for residents in skilled nursing facilities.
  • Coordinate care between residents, providers, facility staff, and family members.
  • Prepare residents for provider encounters and assist with visit workflows.
  • Maintain accurate and timely documentation within the electronic health record.
  • Support transitions of care, referrals, follow-up appointments, and other care coordination activities.
  • Reinforce provider instructions and approved patient education materials.
  • Support compliance, quality initiatives, and administrative functions that contribute to positive patient outcomes.
  • Maintain reliable on-site attendance and professionalism while representing Theoria Medical within partner facilities.

Your Qualifications
Medical Assistants must have completed an accredited Medical Assistant program. Certified Medical Assistant (CMA) certification is preferred. Previous experience as a Medical Assistant in a clinical, post-acute care, care coordination, managed care, or geriatric healthcare setting is preferred.

Physical Demands: Ability to lift up to 20 lbs. independently and assist with resident transfers involving greater weights as part of a team; ability to stand for extended periods; ability to travel to patient locations (e.g., home, hospital, SNF); fine motor skills and visual acuity required.
