Sage Group HealthCare Staffing is seeking RN Hospice for a 13-week travel/contract position in Neenah, WI. Accepting both local and traveler candidates.
Position Details:
Position Name: Hospice Registered Nurse
Location: Neenah, Wisconsin

Contract Length: 13-week contract
Pay Rate: Range $2200 to $2280 Weekly Gross paybased on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health Hospice
Shifts available: Days
Shift timings: 8 Hours; Days, 8a - 5p
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, No call
Position Requirements:
- Registered Nurse: 1 year - Required
- Recent/CurrentHospice Nursing experience: 1 year with the last 3 years- Required
- Home Care Home Base(HCHB) charting experience - Required
- Must have validDriver's License and Personal Auto Insurance
- Must be CovidVaccinated or provide medical or religious exemption
- ASN or an equivalent- Required
- BSN or an equivalent- Highly preferred
- RN License for State applying or Compact if accepted - Required
- BLS Certificationfrom AHA or American Red Cross - Required
- Provide andcoordinate care for assigned hospice patients in comfort of their homes.
- Develop and implementnursing care plans including Start of Care and admissions as assigned.
- Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy.
- Empower patients andfamilies to reach healthcare goals by educating them on disease management.
- Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians'orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
- Ensure patients'status is assessed on an ongoing basis and pertinent information isgathered and documented.
- Notify physicians ofsignificant changes in status.
- Document and completecharting for all patient visits within 24 hours.
- Provide bereavementsupport to patient's families as needed.
- Territories to cover: Neenah, Appleton, Osk Kosh, and Fond Du Lac areas
