Schedule: Pay Per Encounter | Fee-for-Service
Employment Type: W-2 (Minimum 2 days per week)


Job Summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LMHC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.


Key Responsibilities
  • Conduct comprehensive mental health assessments and diagnostic evaluations
  • Provide individual psychotherapy and ongoing treatment services
  • Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
  • Maintain accurate, timely, and compliant clinical documentation
  • Coordinate care with facility staff, medical providers, and other healthcare professionals
  • Adhere to all federal, state, and organizational standards, policies, and regulations

Qualifications

Required

  • Active Florida license in one of the following:
    • Licensed Mental Health Counselor (LMHC)
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Psychologist (PsyD or Ph.D.)
  • Master’s degree (LMHC/LMFT or Doctoral degree (Psychologist) in a relevant mental health field
  • Ability to work independently while collaborating effectively within a clinical team
  • Strong communication and clinical documentation skills

Preferred

  • Experience working with geriatric populations
  • Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings
