Key Responsibilities
  • Conduct comprehensive evaluations and assessments for children facing developmental challenges, ensuring a thorough understanding of their needs.
  • Engage actively in the development and implementation of the Individualized Family Service Plans (IFSP), working collaboratively with families and interdisciplinary teams.
  • Monitor and evaluate patient outcomes, facilitating continuous improvement while contributing to team discussions surrounding care strategies.
  • Provide emotional support and educational resources to families, equipping them with the skills necessary to enhance their child's growth and development.
  • Adhere to both federal and state regulations, maintaining compliance throughout all interactions and documentation processes.
  • Foster genuine relationships with children and their families, creating a safe and nurturing environment conducive to learning and growth.
  • Employ evidence-based techniques and the Natural Learning Environment Practices to facilitate effective support strategies.
  • Regularly communicate progress and concerns to team members to ensure coordinated care and intervention approaches.
Patient Population / Client Focus

The primary focus is on infants and toddlers experiencing developmental delays or disabilities. Our services are designed to provide early intervention support, addressing critical areas such as physical, cognitive, social/emotional, and communication skills during these formative early years.

Qualifications
  • A Masterโ€™s Degree or Doctorate in Social Work.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) through the Department of Public Health.
  • A valid driverโ€™s license is required for potential field visits.
  • Demonstrable expertise in adapting treatment plans to accommodate various learning styles.
