DEPARTMENT:Transitional Rehab Center
POSITION TITLE: TCA Team Lead
RESPONSIBLE TO:Director of Transitional Rehab
POSITION SUMMARY
The TCA Team Lead is responsible for fostering a nurturing, patient-centered environment while ensuring high-quality care and compliance with regulatory standards. This role includes but is not limited to mentoring TCAs, coordinating training, auditing documentation, and collaborating with unit leadership to develop and implement policies and practices aligned with CMS guidelines. The TCA Team Lead serves as a liaison between staff and leadership, promoting teamwork, accountability, and excellence in resident care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Staff Mentorship & Training
Mentor new TCAs during orientation and throughout their employment.
Conduct one-on-one coaching sessions to build staff confidence and competence.
Coordinate and facilitate ongoing TCA training programs.
Oversee completion of TCA training checklists.
Provide guidance and support for staff during daily tasks and challenges.
Participate in the hiring and interview process for TCA candidates.
Ensure staff compliance with continuing education and certification renewals.
Assist with disciplinary action in conjunction with Unit Leadership, ensuring fair and consistent application of policies and procedures.
Charting & Documentation Oversight
Audit TCA (Transitional Care Assistant) charting regularly to ensure accuracy and completeness.
Participate in chart audits and documentation reviews.
Ensure compliance with state regulations and CMS documentation standards.
Scheduling & Staffing Coordination
Adjust TCA schedules as needed to maintain appropriate staffing levels.
Assist with TCA calendar maintenance, vacation coverage, and sick call coverage.
Coordinate assignments and daily task distribution for TCAs.
Ensure adequate coverage for patient care and support during peak hours.
Policy & Practice Development
Collaborate with Unit Leadership to review and create policies and procedures.
Ensure practices align with CMS guidelines and organizational standards.
Participate in state survey preparations and inspections.
Resident Care & Engagement
Routinely round on patients to assess care quality and build relationships.
Participate in community activities, including meal service and social events.
Assist with admissions and care plan meetings.
Monitor and report changes in residents' physical or emotional condition to nursing staff.
Ensure cleanliness and hygiene of resident rooms and common areas.
Support infection control practices, including proper hand hygiene and use of personal protective equipment (PPE).
Schedule
The TCA Team Lead position includes:
Shift coverage requirements in addition to administrative duties every week, which may include training coordination, documentation audits, policy development, staff mentorship in addition to other assigned tasks.
Administrative Support
Maintain inventory of medical supplies and equipment.
Assist with communication regarding facility upkeep and cleanliness.
Coordinate new hire paperwork and employee evaluations.
Assist with completion of annual and new hire mask fittings

QUALIFICATIONS
Minimum Requirements
Prior CNA experience preferred.
Montana CNA certification required.
Current Med Aid Certification or ability to obtain within one year of hire
Skills & Abilities
Strong written, verbal, and observational skills.
Excellent interpersonal and organizational abilities.
Basic computer proficiency.
Ability to work independently and solve problems using sound judgment.
Physical Demands & Working Conditions
Must be able to lift 25 lbs regularly and up to 50 lbs occasionally.
Prolonged periods of standing, walking, bending.
Exposure to outdoor elements, animals, and home-like environments.
Potential exposure to bloodborne pathogens and other healthcare-related risks.
