Supportive Case Managers often work with vulnerable populations, including those experiencing homelessness, mental health challenges, or substance use issues. Their goal is to empower clients to achieve greater independence and stability.

As a Supportive Case Manager, collaboration with other service providers is a core part of the role. You'll regularly coordinate with social workers, healthcare professionals, housing authorities, and community organizations to address clients' diverse needs. Effective communication and teamwork are essential, as you'll be a key link connecting clients to various resources and services.
