Job Title: Medical Director-Post Acute (BC/BE) – Immediate Opportunity

Location: North Carolina and South Carolina

Salary Range: Starting at $275K + productivity and Medical Directorship stipend

Schedule: Full-time flexible Monday - Friday, night and weekend call as needed

Summit Medical Consultants is a physician-founded and led medical group established in Colorado 2015 that is rapidly expanding into the Carolinas. With over 100 clinicians, our mission is to deliver high-quality, compassionate, and coordinated medical care to patients across Skilled Nursing Facilities, Nursing Homes, Long-Term Care, Assisted Living, and Independent Living settings.

We integrate care across disciplines, working closely with Hospitalists, Specialists, Therapists, Case Managers, and Behavioral Health to ensure the best outcomes for our patients.

Position Overview

We are seeking Board-Certified or Board-Eligible Family Medicine or Internal Medicine Physicians for immediate openings North and South Carolina. The ideal candidate will provide compassionate, comprehensive care across post-acute and long-term care settings and collaborate closely with interdisciplinary teams.

Key Responsibilities

  • Provide ongoing medical care and oversight to assigned patients in skilled nursing, subacute, assisted, and independent living settings.
  • Create and maintain timely, appropriate, and confidential documentation in line with medical and billing standards.
  • Possibly provide Medical Directorship service to one facility (that is included in the base compensation). Opportunity to earn extra compensation by taking up additional Medical Directorships.
  • Participate in care coordination with therapists, specialists, and facility staff.
  • Meet quality, safety, and regulatory benchmarks for clinical care and documentation.
  • Support practice-wide utilization and care management goals.
  • Engage in peer reviews, quality improvement, and professional development activities.
  • Build positive relationships with patients, families, facility staff, and the Summit team.
  • Travel between multiple facilities as needed; reliable transportation required.
  • 18+ patients per day on average
  • Share minimal rotating weekend and holiday call duties.

Essential Required:

  • MD or DO from an accredited medical school.
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine.
  • Current and unrestricted medical license in North Carolina or eligibility to obtain one.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Reliable transportation.
  • Minimal weekend call and holiday coverage

Preferred:

  • Familiarity with Point Click Care, eClinicalWorks and Google Suite.
  • Experience in post-acute care, skilled nursing, or hospitalist settings.
