POWER is a mission-driven nonprofit dedicated to helping women reclaim their lives from addiction. We provide trauma-informed treatment and recovery support services for women with substance use and co-occurring disorders, fostering a safe, supportive environment where women can heal, grow, and build lasting recovery.
Why This Role Matters
The Mental Health and Substance Use Disorder Case Manager plays a critical role in helping individuals navigate complex behavioral health and recovery journeys. Through coordinated care, discharge planning, and outcomes-focused case management, this position helps clients achieve stability, improve health outcomes, reduce avoidable hospitalizations, and maintain long-term recovery. By connecting clients to needed services and supports, the Case Manager serves as an essential advocate throughout the continuum of care.
What You'll Do
Care Coordination & Clinical Oversight
- Conduct comprehensive psychosocial and needs assessments for individuals with mental health and/or substance use disorders.
- Develop, implement, and monitor individualized case management plans focused on clinical outcomes and recovery goals.
- Coordinate care across inpatient, outpatient, MOUD/MAUD programs, and community-based providers.
- Collaborate with interdisciplinary teams to ensure alignment of treatment plans and care interventions.
- Monitor client progress and adjust care plans based on changing needs and circumstances.
Outcomes Management
- Track and evaluate progress toward established clinical, recovery, and functional outcomes.
- Utilize data and evidence-based practices to guide interventions and care decisions.
- Identify gaps in treatment and implement corrective actions to improve outcomes.
- Support quality improvement initiatives related to behavioral health and substance use populations.
- Promote recovery-oriented and person-centered approaches to care.
Discharge Planning & Transitions of Care
- Lead discharge planning efforts for clients transitioning from inpatient or higher levels of care.
- Coordinate timely access to step-down services, outpatient appointments, peer support, and community resources.
- Arrange post-discharge appointments and support services to reduce readmission and return-to-use risk.
- Conduct follow-up outreach to ensure adherence to discharge and recovery plans.
- Facilitate continuity of care across treatment settings.
Client Engagement & Support
- Engage clients using motivational interviewing and recovery-oriented practices.
- Provide education to clients and families regarding treatment options, recovery supports, and return-to-use prevention strategies.
- Identify and address barriers to care, including transportation, housing, financial, and access-related challenges.
- Connect clients with community resources and social service supports.
- Foster therapeutic relationships that encourage engagement and positive outcomes.
Utilization Management Support
- Collaborate with utilization management teams to support appropriate level-of-care determinations.
- Participate in case reviews and multidisciplinary discussions.
- Support medical necessity reviews and continuity-of-care planning.
- Assist with coordination of authorizations and service transitions as applicable.
Documentation & Compliance
- Maintain accurate, timely, and compliant case documentation.
- Ensure adherence to organizational, regulatory, and payer requirements.
- Protect client confidentiality in accordance with HIPAA and applicable substance use disorder confidentiality regulations.
- Complete required assessments, reports, and documentation within established timeframes.
What You Bring
Required
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field.
- Minimum of 2-4 years of experience in case management in behavioral health or, substance use treatment.
- Strong knowledge of mental health disorders, substance use disorders, and co-occurring conditions.
- Experience with care planning, discharge planning, and transitions of care.
- Ability to interpret clinical information and apply it to individualized care planning.
- Strong organizational, communication, and problem-solving skills.
- Act 73, 33, and 34 clearances (required for all POWER roles).
- Ability to manage complex and high-risk client situations.
Preferred
- Experience working in managed care, health plans, integrated care, or behavioral health settings.
- Knowledge of ASAM Criteria, DSM-5, and evidence-based substance use treatment models, including MAT/MOUD.
- Case Management Certification (CCM) or similar credential.
- Familiarity with community resources, recovery support systems, and social service networks.
- Experience with quality improvement initiatives and outcomes measurement.

Work Schedule & Environment
- Primarily onsite position.
