We're seeking a dedicated clinician, either a Physical Therapist or Physical Therapist Assistant, who is passionate about delivering exceptional, one-on-one patient care while contributing to a supportive, collaborative team environment. What You'll Do
You will create meaningful patient experiences aligned with our mission of delivering exceptional outcomes through evidence-based practice.
Key Responsibilities
  • Perform or follow initial evaluations in accordance with APTA standards
  • Develop or implement individualized, comprehensive plans of care
  • Establish, follow and monitor short- and long-term patient goals
  • Deliver hands-on, one-on-one therapy to optimize patient recovery and function
  • Track progress and communicate updates with referring providers and care teams
  • Design and document home exercise programs to support ongoing improvement
  • Ensure compliance with insurance and reimbursement guidelines
  • Manage patient discharge planning effectively
  • Maintain current knowledge through continuing education and professional development
  • Communicate professionally with patients, providers, insurers, and other stakeholders
