This Home Health Physical Therapist opportunity is based in the welcoming community of Franklinton, North Carolina and the surrounding areas. Franklinton offers the charm of a close-knit small town, tree-lined streets, and quiet neighborhoods, while being just a short drive from larger hubs such as Wake Forest and Raleigh. Residents enjoy an easy pace of life, a friendly atmosphere, and access to parks, local shops, and community events. The region provides the best of both worlds: a peaceful rural setting with quick access to dining, entertainment, cultural attractions, and educational institutions in the broader Triangle area. Working in home health in this area allows you to build meaningful connections with patients and families right where they live. You will travel to patient homes in Franklinton and nearby communities, supporting individuals who may be recovering from surgery, managing chronic conditions, or working to maintain independence and safety in their daily activities. The setting offers autonomy, flexibility in your day-to-day schedule, and a unique opportunity to see the impact of your interventions in real-life environments. In this role, you will conduct comprehensive initial evaluations in the home, assess functional mobility, strength, balance, and safety, and develop individualized, evidence-based treatment plans that align with patient goals. Typical responsibilities include therapeutic exercise, gait and transfer training, fall risk assessments, home safety recommendations, and instruction in assistive device use. You will provide detailed education to patients and caregivers on home exercise programs and strategies to optimize independence and quality of life. Each day generally includes a series of scheduled home visits, travel between residences, documentation of your findings and interventions in an electronic medical record, and coordination of care with an interdisciplinary team. You will work closely with nurses, occupational therapists, speech therapists, and other clinicians to ensure a cohesive plan of care, timely communication, and strong patient outcomes. Caseloads typically focus on adult and geriatric patients, with visit volumes structured to support thorough, high-quality care and accurate documentation. The environment supports professional growth and the development of clinical expertise in home-based rehabilitation. You will gain valuable experience working across a range of diagnoses, including orthopedic post-operative cases, neurologic conditions, cardiopulmonary limitations, and generalized deconditioning. The role can enhance your clinical skill set and strengthen your rรฉsumรฉ, especially if you are interested in community-based care, population health, and functional rehabilitation. Franklintonโ€™s location provides additional advantages for work-life balance. After your workday, you can enjoy nearby outdoor recreation, local eateries, and the amenities of neighboring towns. With close proximity to the broader Raleigh-Durham area, you have access to continuing education opportunities, professional networking, and a variety of cultural and recreational activities, all while living and working in a more relaxed, community-oriented setting.
