This role is pivotal in delivering high-quality mental health services, specifically tailored for diverse populations in medically underserved communities. The ideal candidate will actively engage with clients experiencing significant mental health challenges, including major mental illness and severe emotional disorders, while promoting recovery and empowerment.

About the Organization

At Stone Path Counseling, our mission is to foster healthier communities by providing safe, patient-centered healthcare services to individuals regardless of their ability to pay. Located in Aurora, Illinois, we are dedicated to serving underrepresented populations and aspire to become a model for health centers across the nation. Our commitment to exceptional care and community outreach drives our efforts daily.

Work Environment

This position offers a remote work setting, allowing you to connect with clients in a flexible manner while maintaining a strong commitment to team collaboration. You will be part of a dynamic community health clinic environment that emphasizes both individual and organizational growth. As a member of our team, you will thrive in a culture that values diversity and promotes innovative approaches to healthcare.

Key Responsibilities
  • Act as a primary clinician, engaging with clients both in virtual sessions and through community outreach initiatives. Conduct assessments and offer therapeutic interventions tailored to each clientโ€™s unique circumstances.
  • Collaborate effectively with team members and other stakeholders to set and achieve shared goals. Participate actively in team meetings, contributing ideas for improvement and innovation in service delivery.
  • Demonstrate cultural competence by respectfully working with clients from diverse backgrounds, including different traditions, beliefs, and interpersonal styles.
  • Adhere to the ethical standards and codes of conduct established by the organization, ensuring all practices reflect professionalism and integrity.
  • Implement and monitor individualized care plans that facilitate access to services and support clientsโ€™ holistic well-being by coordinating with family members and other healthcare providers.
  • Utilize evidence-based practices for brief screenings, therapeutic modalities, and crisis intervention, ensuring clients receive appropriate and timely interventions.
  • Maintain up-to-date documentation of client interactions, treatment plans, and progress to ensure compliance and facilitate quality improvement initiatives.
  • Carry an assigned caseload of clients and tailor services based on their treatment goals, empowering them through choice and active participation in their care.
  • Engage in on-call support, providing assistance outside regular hours as necessary, to ensure continuity of care for clients in crisis.
  • Participate in committees focused on enhancing service delivery and advancing the overall quality of mental health care within the agency.
Patient Population / Client Focus

The client population served by Stone Path Counseling includes individuals from various socio-economic backgrounds who confront challenges related to major mental illnesses and emotional disorders. As an LCSW, you will be instrumental in providing support to those who may not otherwise have access to mental health services, utilizing a client-centered approach that promotes healing and resilience.

Qualifications
  • A Master's degree in Social Work from an accredited institution, enabling licensure as a Clinical Social Worker (LCSW).
  • Strong interpersonal skills that foster rapport with clients and collaborators alike, creating a safe and trusting environment for therapy and support.
  • Expertise in clinical assessment, diagnosis, and treatment, particularly with the populations served by our clinic.
  • Excellent verbal and written communication skills, essential for effective client interactions and documentation.
  • Strong organizational and planning capabilities, allowing for effective case management and service delivery.
Preferred Experience
  • At least 2 years of experience in clinical social work, preferably in community health settings or similar environments.
  • Familiarity with integrated care strategies and the ability to connect clients with diverse care services is highly desirable.
  • Experience in crisis intervention and a strong understanding of mental health treatment modalities.
Compensation and Benefits

This position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience. In addition to financial compensation, Stone Path Counseling provides a comprehensive benefits package which includes health insurance, paid time off, and opportunities for continuing education and professional development.
