Remote Nurse Care Manager (Compact License - Multi-State)
Work Arrangement: Fully Remote
Target Coverage: Multi-State (Compact Licensure States)
Licensure Requirement: Active RN Compact License (NLC) required
Role Overview
A rapidly growing telehealth organization is seeking a Nurse Care Manager to support patients across multiple states through a fully remote care model.
This role focuses on coordinated, patient-centered care delivery, emphasizing chronic condition management, patient education, and collaboration with interdisciplinary teams to improve outcomes and reduce care gaps.
This opportunity is ideal for a nurse who is confident managing patients across state lines and thrives in a technology-enabled care environment.
Key Responsibilities
  • Deliver comprehensive care management services across diverse patient populations
  • Conduct patient assessments and develop individualized care plans
  • Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
  • Provide patient education on conditions, medications, and self-management strategies
  • Use motivational interviewing techniques to improve adherence and engagement
  • Coordinate care with providers, pharmacists, and other healthcare professionals
  • Identify care gaps, risks, and barriers to treatment adherence
  • Ensure accurate and timely documentation in compliance with program standards
  • Support care transitions and continuity across settings

Required Qualifications
  • Active Registered Nurse (RN) Compact License (NLC)
  • Minimum of 2 years' experience in care management, case management, or chronic disease management
  • Willingness to work across multiple state populations

Preferred Qualifications
  • Experience in:
    • Chronic care management programs (CCM)
    • Transitional care management (TCM)
    • Remote patient monitoring (RPM)
    • Telehealth or virtual care delivery
  • Strong background in patient education and engagement
