Work Environment

This role is primarily onsite, allowing you to interact closely with clients in their home environments. Key Responsibilities
  • Direct physical therapy treatment aimed at preventing disability, alleviating pain, and restoring muscle function in alignment with individualized plans of care.
  • Oversee and mentor Physical Therapy Assistants to ensure the effective and appropriate implementation of care plans.
  • Conduct comprehensive assessments to evaluate clients’ functional abilities, including diagnostic tests related to muscles, nerves, and joints.
  • Assess clients' home environments, providing recommendations for modifications that promote greater independence.
  • Administer a variety of therapeutic techniques, including manual therapeutic exercises, traction, and mechanical therapies.
  • Evaluate and modify the use of prosthetic and orthotic devices where necessary, collaborating closely with physicians.
  • Maintain accurate clinical documentation, including OASIS assessments, progress notes, and discharge summaries, to support client care plans.
  • Actively participate in interdisciplinary team meetings to discuss client treatment goals and updates, ensuring a thorough and collaborative approach to care.
  • Educate clients and their families about treatment regimens, physical therapy devices, and strategies to facilitate ongoing recovery and health maintenance.
  • Perform site visits to assess the usage of community resources for rehabilitation and support for clients.
Patient Population / Client Focus

Your primary focus will be on clients within the home care setting, ranging from those recovering from surgery to individuals managing chronic conditions. By recognizing the diverse needs of your patients, you will aim to enhance their mobility, independence, and overall quality of life through tailored therapy interventions.

Qualifications
  • Bachelor’s degree in Physical Therapy (DPT) is required.
  • Current licensure as a Physical Therapist in good standing in the state of Florida.
  • Minimum of two years of clinical experience in acute care or home care settings.
  • Certification in CPR and a valid driver’s license with automobile insurance.
