Job Description: Provide primary health care services to the community. Implement therapeutic interventions (including pharmacotheraputic) for chronic, stable & common conditions. Utilize and synthesize appropriate health assessment & differential diagnosis of client/patient status. Assess need for ordering and interpreting appropriate lab, radiology and ultrasound diagnostic tests. Work in conjunction with the client, family and health providers to develop & implement holistic treatment, interventions & support. Be part of a diverse interdisciplinary team working in the Delisle area. Provide educational support for nursing, medicine & other disciplines students. Support & participate in multi level chronic disease management strategies. Assist individuals, families & communities to increase knowledge, control & influence of health determinants in the maintenance and improvement of their health status. Work with health care, community and intersectoral partners. Support & assist the development/implementation of quality initiatives to benefit service delivery models. Contribute to research activities.

Experience

  • Three (3) years full-time experience as a Registered Nurse

Education

  • Master of Nursing - Nurse Practitioner (MN-NP)

Other Education and Training

  • Nursing diploma is acceptable
