Job description

We are currently seeking a Social Services Designee.

Job Responsibilities and Duties:
  • Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department
  • Assist with facility admissions
  • Manage resident trust accounts and assist with resident shopping
  • Schedule resident appointments and coordinate with other departments as needed
  • Maintain accurate charts and documentation of resident care
  • Engage, converse, and interact with residents to promote a positive environment
  • Work primarily during daytime hours, with occasional evening and weekend visits as required
Requirements:
  • Prefer 2-5 years of Social Work experience, ideally in Long Term Care setting.
  • At least 1 year of supervised social work experience in health care setting working directly with individuals including those with mental challenges.
  • Knowledge about Medicaid and Medicare programs is a plus.
