Job description

Faith Home Health & Hospice is seeking a compassionate, skilled Speech Therapist dedicated to the highest standards of care and professionalism. Join our Topeka team and make a meaningful impact on patients’ lives.

Position Overview

As a Speech Therapist with Faith, you will provide one-on-one speech therapy using individualized care plans tailored to each patient’s needs. You’ll collaborate with a supportive team of healthcare professionals who share your commitment to quality care.

Job Summary

Provide PRN speech therapy services to patients as directed by the plan of treatment and in accordance with the philosophy, objectives, and policies of the agency.

Responsibilities
  • Develop or assist in developing a written plan of care for each assigned patient, assessing physical and psychosocial needs, and documenting findings.
  • Assess patient’s condition, initiate plan of care, and reevaluate/update as necessary.
  • Evaluate assessment findings and communicate pertinent information about the patient’s clinical status and ongoing needs in a timely manner.
  • Submit verification of completed visits per agency policy.
  • Promote patient/family/caregiver education using verbal and written communication techniques that respect cultural, ethnic, and personal preferences.
  • Qualifications

    Current state license as a Speech Therapist.

    Graduate of a Speech Therapy curriculum approved by the American Speech-Language-Hearing Association or the Council of Medical Education and Hospitals of the AMA.

    Minimum of one year of experience as a Speech Therapist.
