Position Overview

Spectrum Wellness and Mental Health Group is seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote role. In this position, you will have the opportunity to leverage your clinical skills and knowledge to support individuals, couples, and families in navigating complex mental health, substance abuse, and medical challenges. This role is instrumental in providing quality behavioral health services within our Hospice Center, ensuring that patients receive the comprehensive support they need during critical stages of their lives.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments that consider age, cultural background, and individual unique circumstances to evaluate mental health, substance use, and medical needs.
  • Deliver tailored behavioral health interventions, including individual and group counseling, treatment planning, crisis intervention, and referrals as needed, ensuring that each client receives the appropriate level of care.
  • Document clinical interactions thoroughly and efficiently using electronic health records, including notes on assessments, progress, and any changes in patient conditions.
  • Work collaboratively with Quality Improvement teams to implement data collection processes that enhance service delivery and patient outcomes, driving evidence-based practices within the organization.
  • Provide social work support and linkage to community resources, ensuring clients have access to medication assistance programs and other necessary supports.
Patient Population / Client Focus

As an LCSW at Spectrum Wellness, you will primarily work with a diverse population in hospice care, enabling patients during vulnerable times of their lives. You'll provide support not only to individuals facing terminal illness but also to their families, facilitating discussions around grief, care plans, and mental wellness. Qualifications
