Spectrum Healthcare Services is recruiting for a Nurse Practitioner or Physician Assistant for a family medicine practice in Louisville, KY.
Essential Duties & Responsibilities
  • Conduct comprehensive patient assessments, including medical histories and physical examinations, for acute and chronic health conditions.
  • Order, interpret, and evaluate laboratory tests, diagnostic studies, and other clinical data to support diagnosis and ongoing patient care.
  • Develop and implement individualized treatment plans utilizing evidence-based preventive, acute, and chronic disease management strategies.
  • Prescribe medications and other therapeutic interventions in accordance with state regulations, practice protocols, and collaborating physician guidelines.
  • Perform patient follow-up visits and reassess treatment effectiveness, modifying care plans as clinically appropriate.
  • Educate patients and families regarding disease prevention, health promotion, treatment options, lifestyle modifications, and wellness goals.
  • Coordinate care with physicians, specialists, ancillary providers, and other members of the healthcare team to support continuity of care.
  • Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).
