Registered Dietitian

Complete Care at Southpointe – Greenfield, WI

Complete Care at Southpointe is seeking a highly skilled and dedicated Registered Dietitian to join our compassionate interdisciplinary team. In this role, you will be responsible for planning, organizing, and directing the comprehensive nutritional care of our residents in accordance with federal, state, and facility standards.

Major Duties and Responsibilities

  • Nutritional Care & Assessment: Assess and monitor residents' nutritional status, complete thorough nutritional assessments (upon admission, quarterly, annually, and with condition changes), and accurately complete assigned sections in the Minimum Data Set (MDS).

  • Care Planning & Monitoring: Develop, implement, and routinely update nutritional care plans. Monitor residents for weight changes, nutrition support needs, and skin breakdown to provide timely medical recommendations.

  • Menu & Meal Oversight: Oversee diet orders, review menu changes for compliance, and observe meal services to ensure appropriate therapeutic modifications and correct textures are served.

  • Collaboration & Education: Partner with the interdisciplinary team to align diets with specific medical conditions, and provide essential nutritional education to residents, families, and staff.

  • Compliance & Auditing: Perform regular inspections of food service areas for sanitation and safety, conduct routine care audits, and actively participate in facility inspection surveys.

Required Qualifications

  • Education: Bachelor's degree in Dietetics from an accredited college or university.

  • Licensure/Registration:

    • Registered by the Commission on Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics.

    • Actively licensed in Nutrition/Dietetics in the State of Wisconsin.
