Job description

SUMMARY

The Licensed Practical Nurse is responsible for participating in assessing, implementing and documenting resident care as ordered by a physician and with the continuous availability of a registered nurse.

ESSENTIAL DUTIES AND RESPONSIBLITIES include the following (other duties may be assigned):

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse.
  • Performs triaging via telephone and in-person, in clinic or on The Sharon at SouthPark premises, of residents and employees. Takes further action as findings indicate.
  • Demonstrate the ability to handle emergency situations in a prompt, precise, professional and safe manner. Determine the appropriate interventions and notify family, physician and RN supervisor of event. Complete incident report and applicable follow up form completely, and accurately in a timely manner following event.
  • Prepares and administers medications, tests and treatments accurately, observing patient response, as ordered by the physician and documents outcomes timely and appropriately in the medical records.
  • Participates in on-going patient assessment and evaluates patient’s responses to nursing actions under the guidance of a registered nurse.
  • Documents changes in patient conditions and notifies physician, responsible party and supervising RN of changes.
  • Completes appropriate documentation and other record-keeping duties as per company policies and relative local, state and federal regulations in patient’s electronic medical records (Point Click Care).
  • Communicate with internal and external physicians regarding patients care and condition.
  • Transcribes, clarifies, co-ordinates and implements physician orders.
  • Communicates with the residents, physician, family members, and other members of the health care team.
  • Complies with company policies, procedures, and related local, state and federal regulations.
  • Maintain accountability for controlled substances and inventories medications at the beginning and end of each shift with another licensed nurse. Notifies RN supervisor of discrepancies.
  • Supports the facilities medical supply system, follows facility procedure for patient charge items. Follows clinic fee schedule with regards to patient charges for time and services rendered.
  • Remains current on nursing techniques and advances as applies to LPNs by attending in-services, workshops, seminars, and continuing education opportunities. 100% attendance to all mandatory in-services/competencies yearly is required.
  • Ensures supplies and equipment necessary for patient care are stored in an organized, safe and efficient manner.
  • Communicates and co-ordinates resident medication orders with Pharmacy as needed, per physician’s order.
QUALIFICATIONS:

  • Ability to deal effectively with confused and/or agitated residents.

EDUCATION AND EXPERIENCE:

  • High School Diploma required.
  • Graduate of an accredited school of nursing.
  • Must have a LPN license with the board of NC Nursing or compact license.
  • Prior customer service experience preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Current unencumbered North Carolina Licensed Practical Nurse license required.

REASONING ABILITY:

  • Ability to identify special resident problems and report them immediately to supervising nurse.
  • Ability to identify safety hazards and emergency situations, initiate corrective action as necessary and/or seek assistance of supervising nurse.
  • Recognition that errors in judgment may jeopardize the safety of the residents.
  • May be called upon to be a resource person for a new team member.
  • Works under limited supervision of a registered nurse and facility policies and procedures.
