Job description

Title: LPN – Long Term Care (LTC)
Duration: 13 Weeks (Contract Assignment)
Shift: 12-Hour Days (6:00 AM – 6:00 PM), 36 Hours/Week
Facility: Southeast Colorado Hospital District
Location: Springfield, CO

Responsibilities:
Provide direct and indirect nursing care to LTC residents, administer medications and treatments, monitor resident conditions, document care accurately, communicate with physicians and families, supervise CNAs, and support daily resident care operations. Must have an active LPN license, BLS certification, and at least 1 year of LTC experience. Cerner EMR experience preferred.
