Job description

Job Title: Per Diem Physical Therapist

Company Name: Southbridge Rehabilitation & Health Care Center

We are seeking a per diem Physical Therapist to join our dynamic care team at Southbridge Rehabilitation & Health Care Center. As a Physical Therapist, you will provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload. You will also participate in the clinical team process, provide teaching and training to caregiver staff, and supervise other therapists, assistants, and aides.

Up to $55 an hour with weekly pay.

Responsibilities:

  • Screens patients/residents and evaluates them to develop appropriate care plans

  • Provides direct resident treatment in accordance with an established plan of care

  • Contributes to the team meeting and decision-making process

  • Trains patients, residents, and caregivers in various aspects of care

  • Monitors efficiencies and documents activities according to the company's clinical documentation system

  • Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations

  • Communicates effectively with patients, residents, team members, customers, and supervisor

  • Participates in clinical program development and implementation

  • Participates in the training and orientation of other staff, students, and volunteers

  • Participates in census building and marketing activities

  • Participates in quality improvement activities

  • Complies with facility Privacy Practices policies and procedures and protects residents' individual health information

Requirements:

  • Bachelor of Science Degree, Master of Science or Doctoral degree in Physical Therapy

  • Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers

  • Strong oral and written communication skills

  • Strong organizational and analytical skills

  • Ability to perform documentation needs, manipulations of equipment, and direct patient care

  • Requires intense attention and concentration

  • Ability to cope with periodic stress associated with changing census and staffing demands

  • Demonstrated ability to prioritize and manage time effectively for self and others

  • Ability to deal with a wide range of personality types, ages, and diverse cultural backgrounds

  • Ability to articulate and comprehend the English language

  • Standing and walking for significant amounts of time is necessary
