Job description

Interim HealthCare is seeking an organized, compassionate, and dependable Care Coordinator to ensure our clients receive consistent, high-quality care.
Key Responsibilities:
  • Manage daily schedules and ensure all client shifts are properly staffed.
  • Confirm caregiver attendance and review daily care notes.
  • Respond promptly to scheduling conflicts and service concerns.
  • Speak with incoming leads and explain available home care services.
  • Complete initial in-home assessments and client visits.
  • Coordinate first shifts and follow up with clients and families.
  • Maintain accurate client documentation in AxisCare.
  • Communicate closely with caregivers, supervisors, clients, and families.
  • Build relationships with prospective clients and referral sources.
  • Ensure compliance with care plans, agency policies, and state requirements.

The ideal candidate is a strong communicator and problem-solver who can manage multiple priorities in a fast-paced environment. Home care, healthcare scheduling, client services, or care coordination experience is preferred.
