Job description

The Social Worker is responsible for providing psychosocial support and services to residents and their families. This role ensures compliance with federal, state, and local regulations while supporting resident rights, care planning, discharge planning, PASARR compliance, quality of life initiatives, advanced directive facilitation, and coordination of community resources to promote residentsโ€™ well-being, dignity, and quality of life. The Social Worker collaborates with the interdisciplinary team to address emotional, social, behavioral, and environmental factors that impact resident care, outcomes, and successful transitions.

Responsibilities:

  • Supervises psychosocial support and services provided by Social Worker and Social Services Designee.
  • Carries out supervisory responsibilities in accordance with the organizational policies and applicable laws.
  • Interviews, recommend hires and trains employees
  • Plans, assigns, and directs workflow.
  • Prepares work assignment sheets, taking into consideration resident safety, the educational preparation, experience, knowledge and ability of the persons to whom the assignments are made.
  • Receive calls and find replacements for absent staff as needed
  • Transfers employees within units as needed.
  • Prepares performance evaluations with the understanding that such evaluations impact tenure of probationary employees and wage increases of non-probationary employees.
  • Schedules and adjusts lunch and rest breaks.
  • Receives and resolves employee complaints.
  • Monitors and corrects job performance of employees.
  • Adequately supervises assigned staff and assures that those staff are supervising employees assigned to them.

Education and/or Experience:

  • Minimum of bachelor's degree in social work or bachelorโ€™s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
  • One year supervised social work experience in a health care setting.
  • Working knowledge of federal and state long-term care regulations.
  • Knowledge of chronic disease management and care transitions/discharge planning is preferred.

Certificates, Licenses, Registrations:

  • Current Social Work license in the state as required by state regulations.
  • LSW, LMSW, LCSW, LBSW, LLBSW, or LLMSW preferred.
