Job description

  • Conduct formal intake interviews; make assessments and referrals as needed.
  • Complete psycho-socials and social history when applicable.
  • Provide short and long-term counseling, crisis intervention, safety planning, and psycho-education.
  • Maintain complete, accurate, and up-to-date programmatic paperwork and statistical information.
  • Assist clients in completing entitlement forms as needed.
  • Make referrals for various services, such as food, clothing, bathing, psychiatric evaluations, medical evaluations, TB and HIV testing, and accompany clients as needed and also handle follow-up as needed.
  • Make referrals to other service areas within Project Hospitality and the community.
  • Participate in weekly supervision, weekly staff meetings,
  • Facilitate community meetings/groups and ongoing case conferences.
  • Transport/Accompany clients to appropriate referral sites when deemed necessary.
  • Masters Degree in Social Work and LMSW required.
  • Minimum of 1-year experience working with the mentally ill and homeless populations, experience with substance abuse also preferred.
  • Experience with group work, and working with individuals needed.
  • Knowledge of DSM V.
  • Bilingual Spanish/English is a plus.
  • Must have exceptional communication skills both orally and in writing, with individuals and groups regarding confidential and sensitive issues.
