Job Description
Saint Luke's North Hospital is seeking a PRN Social Worker to join our team!
Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient s health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participates in program planning and development to meet the needs of customers within the health care system and community.
  • PRN, shifts available any day of the week, hours are 8 a.m. to 4:30 p.m.
  • Completes family therapy sessions with patients and their families.
  • Participates in daily treatment team meetings.
  • Provides psychoeducational group therapy sessions to patients.
  • Assists with discharging planning, making referrals and assisting with transportation arrangements
  • Master's in Social Work required
  • LMSW (or LCSW) in MO required
