Job description

The Social Worker II is a key position responsible for delivering advanced clinical services, guiding case management strategies, and supporting program development for formerly homeless senior residents. This role combines direct client services with clinical services ensuring high-quality, trauma-informed, and person-centered care throughout our housing programs.?This is a full-time, temp to perm role in Manhattan.

Job Duties & Responsibilities

  • Provide intensive case management and therapeutic support to a caseload of high-need?residents.?
  • Conduct psychosocial assessments, develop individualized service plans, and oversee implementation.?
  • Offer crisis intervention, mental health support, and linkage to substance use treatment when needed.?
  • Assist?residents in accessing entitlements, medical and mental health care, home care, and other community resources.?
  • Facilitate group sessions to promote wellness, socialization, and skill-building.?
  • Serve as a resource for complex case consultations and multidisciplinary care planning.?

Salary Range: $62,000-$68,000.
