About the Role:
The Social Work Assistant supports the Care Management team by assessing patient needs, addressing social determinants of health, and connecting patients with community resources and services. This role collaborates with case managers to promote safe care transitions, advocate for patients, and support positive health and social outcomes.
Key Responsibilities:
  • Conduct initial patient assessments, identify social and resource needs, and document interventions and patient interactions accurately and ethically.
  • Coordinate community resources and support services to address social determinants of health, including access to healthcare, housing, transportation, employment, and follow-up care.
  • Assist with care planning, patient advocacy, and emotional support while collaborating with case managers and service providers to ensure patients receive appropriate resources and services.

Requirements:
  • Education: Bachelor of Social Work or Arts required. Student in an accredited master's degree in Social Work program required.
  • Experience: 1-2 years of acute care hospital experience and medical terminology.
